Then you don't need to add salt, if it is a low-end BW fish, just heat & large water changes. What fish is it?

[welcome] Those black spots are it's tiny spines. That salt you are using is the totally wrong salt. Softer water is the opposite of you want. You MUST use marine salt, like Instant Ocean. Neither the raphael, nor the pleco should be in salt. Also, the puffer will nip both of those fish very badly. ...

I don't really think you ever need Purigen or carbon in there. I always keep Melafix on hand. It's very helpful for it's healing properties. Try to keep nitrate <20 at all times.

Yes but if you have a large enough filter, that shouldn't matter much. It actually adds extra filtration. Rinse the sponge, when flow starts to slow down.

How was the tank cycled? Is it possible he was sickly before this happened, which is why he couldn't swim away from the filter?

They actually can heal fairly well from this. Tell me more about his main tank? Last thing I'd do is move him to an uncycled tank.
