Lip "out of joint"? Severum; advice from my Puffer friends?

Hi guys - problem in one of my other tanks - sadly, my Redheaded Severum isn't doing well. Over the past week or so, I thought he might have been fighting with the other fish (large Jade Goby, Parrots, Geos, Silver Dollars, all nice quiet fish - no agression). But he stopped eating and his lips look like they're "out of joint" - see in the picture, they look odd. And he has a few scratches on his head now. What do you think? I've had him for about 4 years, always been healthy and happy.

That looks terribly uncomfortable. I think PP might be able to help on this though.

What a beautiful fish! It may just be swollen, I'd stick with Melafix for now but Epsom salt may help with swelling.

Thanks for the replies. No, that wasn't Hole in the Head, that was marks due to his thrashing around the tank (in pain, I think). Sadly, he died the day after I posted this. He was a really handsome fish.

Well, he wasn't the most playful fish in the tank - so he was just "Red Head".
