Not good. The overload of waste in that tank must have been what poisoned him. I would not get another one tomorrow if this one doesn't do well especially if its from the same tank.
Try brine shrimp and what about his eye: fuzzy? translucent film?

Read this before posting!!
Since this board has been up, we have found there are several questions that routinely get asked in order to help diagnose problems. If you can have that information to begin with in your post, we'll be able to help right away (if we can!) without having to wait for you to post the info we need.
1) Your water parameters - pH, Ammonia, Nitrite, Nitrates and salinity (if appropriate). This is by far the most important information you can provide! Do not answer this with "Fine" "Perfect" "ok", that tells us nothing. We need hard numbers.
2) Tank size and a list of ALL inhabitants. Include algae eaters, plecos, everything. We need to know what you have and how big the tank is.
3) Feeding, water change schedule and a list of all products you are using or have added to the tank (examples: Cycle, Amquel, salt, etc)
4) What changes you've made in the tank in the last week or so. Sometimes its the little things that make all the difference.
5) How long the aquarium has been set up, and how did you cycle it? If you don't know what cycling is read this: Fishless Cycling Article and familiarize yourself with all the information. Yes. All of it.
We want to help, and providing this information will go a LONG way to getting a diagnosis and hopeful cure that much faster.
While you wait for assistance:
One of the easiest and best ways to help your fish feel better is clean water! If you are already on a regular water change schedule (50% weekly is recommended) a good step to making your fish more comfortable while waiting for diagnosis/suggestions is to do a large water change immediately. Feel free to repeat daily or as often as you can, clean water is always a good thing! Use of Amquel or Prime as a dechlor may help with any ammonia or nitrite issues, and is highly recommended.
Note - if you do not normally do large water changes, doing a sudden, large water change could shock your fish by suddenly changing their established water chemistry. Clean water is still your first goal, so in this case, do several smaller (10%) water changes over the next day or two before starting any large ones.

well it kind of looks a spot of white cloud hmmm not sure how to say it i guess it looks like split
Edit: and how do we tell if the puffer fish has over grown teeth

Hi Sharkbait.
First of all, there is always one thing to concentrate on when you have a sick fish, and that is clean water. If you don't regularly do water changes, start doing them now, small ones at first, moving up to larger ones.
Puffers do not handle dirty water at all, and develop a ton of issues from it. Since you don't know your nitrates, its hard to tell if that is the problem.. and FYI, a biowheel that is doing its job properly will only produce nitrates FASTER, it doesn't remove them. Only water changes can do that (or live plants, but at a much slower rate).
Do not be surprised if this puffer does not survive, but do try to bring it around by keeping the water extra clean (daily water changes if you are able). If you are able to get pictures, let us see what it looks like, so we can get an idea if the eye problem is fungal, bacterial, etc.
What is also a very likely problem is internal parasites. GSP's are all wild caught and usually have some parasites, and the stress of being caught/shipped/displayed/moved/new tank setup can really lower their immune system, allowing the IP's to really take advantage. There is a point that once the infestation has gone too far, the fish just stops eating and doesn't recover (you can try deworming via water treatment but it often doesn't work if the fish has reached this point)... your puffer MIGHT be at that point, but it also might not.. get the water as clean as possible and see if his appetite returns.
Bloodworms are a great idea.. just be sure you are cleaning up any uneaten food so it doesn't pollute the tank more.

i do water changes alot 30~50% every week so i know the water is clean but i think its not cause of my tank.....

Most of us do 50% minimum every week, unless our tanks are understocked. You don't know that your water is clean until you test it.

Lilgreenpuffer XD i did test the water it came out clean
but they didnt tell me my nitrates
and i tryed everything he just doesnt eat
