How was his belly after feeding when he was acting healthy? Did his belly go down like it was sucked in some, the day after feeding? IMO that is a good sign of IP's aside from seeing them come out of the puffers behind. John have you read the article for treating IP's in the library? If not here it is: library/hospital/internal-parasites-pre ... treatment/
Your MBU has a few of the signs of IP's. He is not well either way. You are fighting a hard battle.

Read this before posting!!
Since this board has been up, we have found there are several questions that routinely get asked in order to help diagnose problems. If you can have that information to begin with in your post, we'll be able to help right away (if we can!) without having to wait for you to post the info we need.
1) Your water parameters - pH, Ammonia, Nitrite, Nitrates and salinity (if appropriate). This is by far the most important information you can provide! Do not answer this with "Fine" "Perfect" "ok", that tells us nothing. We need hard numbers.
2) Tank size and a list of ALL inhabitants. Include algae eaters, plecos, everything. We need to know what you have and how big the tank is.
3) Feeding, water change schedule and a list of all products you are using or have added to the tank (examples: Cycle, Amquel, salt, etc)
4) What changes you've made in the tank in the last week or so. Sometimes its the little things that make all the difference.
5) How long the aquarium has been set up, and how did you cycle it? If you don't know what cycling is read this: Fishless Cycling Article and familiarize yourself with all the information. Yes. All of it.
We want to help, and providing this information will go a LONG way to getting a diagnosis and hopeful cure that much faster.
While you wait for assistance:
One of the easiest and best ways to help your fish feel better is clean water! If you are already on a regular water change schedule (50% weekly is recommended) a good step to making your fish more comfortable while waiting for diagnosis/suggestions is to do a large water change immediately. Feel free to repeat daily or as often as you can, clean water is always a good thing! Use of Amquel or Prime as a dechlor may help with any ammonia or nitrite issues, and is highly recommended.
Note - if you do not normally do large water changes, doing a sudden, large water change could shock your fish by suddenly changing their established water chemistry. Clean water is still your first goal, so in this case, do several smaller (10%) water changes over the next day or two before starting any large ones.

if his belly is dark he really isnt well. normally i would expecxt a white / occasionally slighly yellow belly with darker skirt and usually a dark ring around his beak / mouth. i would start feeding him on apple snails QT'd and or frozen shellfish purchased from your local fish counter its cheap enough my large mbu will eat about 4-7 prawns every 2 days and the baby 1/2 king prawn per day so a bag lasts a few weks for both. i buy live missels freeze them, frozen king prawns squid etc etc just vary there died occasional apple snails are dropped in also which i buy aand keep usually at room temp 20degrees in a 5L plastic goldfish bowl feed them carrot until red comes out so i know there systems are fllushed then drop a few in as occasuional weekly treats.

Hi Folks,
I really need help, my little guy looks pretty bad. Any ideas on what I could treat him with, he is kind of hanging near the filter at the top of the tank or near the bottom of the tank-just sitting there. He is dark and has some scratches from the rocks. Any urgent action I can take-I'm at a loss. Help

being only 6" at his age is very bad to be honest all mbus ive kept by hs age are over 1 foot if not more. i sugest get as much air into tank as possible. subdue the lighting or if you have night blue lights / leds use those to settle him. De worming again would be a good thing. as my above post i strongly suggest changing his diet.
trick is keep it well airiated with air pumps with low lighting to settle him. and hope he can hang in there.
you never posted parameters so i think thats a start aswell get back with tank size + capacity, tankmates, perams of water i.e. temp PH KH Ni Na Am.
what water changes do you do? how often do you treat the water use RO etc.
