Maybe you should dismantle your filter and clean it out with tank water. Could be something rotting in there and leaching an overwhelming amount of ammonia?

Even though you need to treat for IP's, treatment is stressful for fish. Especially if they were showing symptoms in the first place. That might be what his problem is.
