Puffers are designed to puff up by sucking in water, not air. If taken out of the water, or startled when at the surface, they can sometimes puff up with air instead. This is bad, because they can't expel the air as easily as they could with water, so they get "stuck".

Then they need us to help by "burping" them, basically holding them under water, nose up, tail down, and shaking them gently or lightly rubbing their belly to get them to release the air easier.

They can die if they don't release the air, so its pretty important to take care of it right away.
