Porcupine puffer grey"ish" spot and swollen

Help Please!
I just put the porc. in the hospital tank, what kind of med's should I use? Has anyone ever experienced anything like this? His right side and under his eye has a large grey patch that goes all the way down his "belly", and his jaw area is swollen. He looks sad, but he is still eating a little. He is also swimming FAST and spastic.
PLEASE, PLEASE PLEASE help.
Ammonia is 0
Nitrates 20 (higher than usual)
Ph 8.2
