I notice this in the last 3-4 days, I was wondering if this is an ich outbreak.

Currently he's in the tank by himself with a blue damsel, I couldn't see any white spot on the damsels, but he's getting more and more daily.

Current Setup: FOWLR
Ammonia - 0
Nitrite - 0
Nitrate - 5
SG 1.020
PH 8.2
Calcium - 380
Alk - 8 DKH
Temp - 79-80 degree F

If this is an ich, then I'll start hypo on him and leave the main tank fallow for 4-6 weeks. I just need to confirm. I do have another tank setup for QT ready to go.

Yup, that's SW ich.

Straight from the Queen...Awesome.

I'll move him to Hypo tank tonight then..I just need some assurance. He's been eating like a pig as usual. I always dose the food in zoe and garlic every feeding. Eats every night between 1-2 silversides, a half of clamp, squids and half of market shrimp.

How big is this fish. May not be necessary to feed daily, unless it's a juvie. Doesn't look chubby though.

Age is unknown, but he's about 3" long. I suspect he's a juvie.

Any other fish that were in the origingal (display tank) with the puffer??

As far as fish, just a blue damsel. I will be Hypo'ing the damsel together with the puffer as well in a 30 gal tank

There's some crab, peppermint shrimp and few hermit..

That is good.
Make sure that you leave the display tank fallow for a minimum of at least 6 weeks - 8 is better!
Do not add anything to that tank during this period and make sure to quarantine everything before placing in the display tank.
