Anchor Worm

Title says it all. I have had this nasty worm in my 46 gallon bow front freshwater tank for months now. I have tried a few medicines but had no luck. Currently a blue marble gourami has it and two plecos. Any feedback on how to get the tank better would be greatly appreciated!
