Greenspot puffer lump behind fin air bubble?

I have a 4-year-old GSP. I've had him/her since it was the size of a quarter. He's about 4 and 1/2 - 5 in Long now. He developed this lump 3 weeks ago, I've tried 15 minute Epson salt bath, I've tried burping via online methods, holding him vertical moving him up and down while gently squeezing the air bubble to no avail. Anyone ever had this happen? Would a syringe in needle help to draw the air out?
Water parameters are all perfect vai api liquid test kit, brackish water. 1.010 gravity temp 78. Tank is over 4 years Old.
Diet is varied, Ram's Horn snails, Frozen peas, Frozen brine shrimp, Frozen blood warms, cooked shrimp, occasional nightcrawlers.
He has eaten at least 30 peas or more since the lump has been.
Is not floating to the top of the water. He is just remaining head down air bubble up and swimming. Funny when he tries to come for food.
Any help would be greatly appreciated!
