SAP with Swelling and White Spot

Hi All,
Lurker here for a while -
I recently set up a tank for my South American Puffers and need help identifying what's going on with one of them. It's in constant motion so it's hard to get a great picture but it has a swollen "head" slightly towards its side with a small white protrusion coming out of an indent in the swelling.
This came on pretty quickly as all the fish in the tank were looking great yesterday. However, I did start treating for parasites yesterday with API General Cure (before I read the post a few down on why API GC isn't great). For more context, almost all of them got ich the first week in the tank but recovered quickly after treatment and I assume that most of the came in with parasites as well.
My plan right now is to finish the API General Cure course tomorrow morning and if it's not improving start treatment with Levamisole HCl or potentially a fungal treatment but I haven't been able to diagnose the issue and was hoping the folks here might be able to provide some guidance.
Other than the swelling and white spot it's acting normally and eating well (Mysis and Bug Bites).
