Injury or sickness?

Hello,
I have a pea puffer with a whitish lump that I don't know if it's an injury or an illness. I haven't seen any aggressive behavior, just posturing and short chases. It's acting normal, eating, and everything else in the tank is fine.
29g, 40g filter
ph 7.4
Ammonia 0ppm
Nitrate 0ppm
Nitrate 0ppm
Temp 77°
9 pea puffers
6 otocinclus catfish
Approx 3 doz shrimp (amano, ghost, and cherry)
Pest snails
Heavily planted tank
Feed 2x day small variety of baby brine shrimp, frz bloodworms, grindal worms, snails
Water change 1x week
No changes in the last week
Tank set up 3 months cycled with live plants, filter medium from established tanks, once algae developed
