Need help

Hi guys so I need some help I have a gsp that’s in a 60 gal. tank I have noticed the last few day he has been staying to the back of the tank just surfing with a grey belly. He is still eating but he is just not as active when it comes to us being around the tank he would always come to the front of the glass when we would enter the room. Today I noticed that his eyes have became cloudy. I do about a 45-50% wc every week and the salinity is at 30%. So I need some help I don’t know what to do I don’t want him to die or be sick any advice
