Spotted Congo sunken belly

Nitrate:20 ppm
Tank mates: fronts and yellow calves, green phantom pleco, vampire pleco, clownfish pleco
Just added stress coat last night and Prazipro
Tank is 100 gal no changes but a small water change, had tank established for several years and he’s Been in tank for just about a year
Spotted Congo staying close to substrate, not moveing, not eating, not being his normal social self, just started yesterday, usually feed him frozen bloodworms, or clams on half shell frozen. Please help not sure what to do
