Need help identifying cyst? On fahaka puffer

Three days ago I noticed a bump the size of a dime on my fahaka underbelly. In the last 3 days it has grown into what looks like a cyst. He is not eating and is hanging out at the top of the tank. Water parameters are all on point. Hes in a 125 gallon brackish tank and hes been an extremely healthy fish for 2 1/2 years. Hes currently 13" long and until he stopped eating was nearly 7" around. I've searched everywhere for answers but cant find out what this is to even treat it. Started treating the tank with paraguard with no improvement. Wondering if its internal parasites or possibly a tumor of sorts.
