takifugu w/ large spots

We have got a takifugu in the store right now and i was curious if you guys may have had experience with this type of disease. To me it looks like really large ich spots. they are very uniform dots, about the size of a tiny egg. Columnaris came to mind but im not sure if puffs are really susceptible to that or not. Plus ive never seen that particular disease in such tiny perfect little bb's. Its only on the fins and tail right now. The water tested perfect with very low nitrates.

My main question is what kind of medication would you use on such a touchy puffer?

Do they seem to cover all of him in a very uniform pattern, if they do its probably his spines, they look like little white dots when they're not puffed up.

I don't thik it's spines if they are only on the tail and fins.
