Fahaka Upside Down

1)
Ammonia: 0
Nitrite: 0
Nitrate: ~ 10-20
2) 50 Gal with just the 1.5in-2in Fahaka.
3) Feed Mysis/bloodworms once every 2 days.
4) no changes
5) tank has been set up for 6 months.
I have no idea what's wrong with this puffer, he swims around but is completely upside down. I've tried to "burp" him, I've applied epsom salt, I've left him alone... idk what else to do. It's been 2days since he last ate so I fear his end is coming soon. Any info on what could be wrong or how to fix it would be appreciated!
