Figure 8 // Sunken Belly

One of my 3 figure 8 puffers has a very sunken stomach. He eats a lot, but within 10 minutes his belly shrinks again.
Aquarium has been running since March of 2016
Ammonia - 0.5ppm
Nitrite - 0ppm
Nitrate - 30ppm
Salinity - 1.002
55 Gallon - 3 Figure 8 puffers.
Feeding shrimp and snails. 20% Water Changes every week. Added SafeStart when cycling, use prime during WCs.
All other puffers are fine, I am thinking he may have parasites, if he does, how should I setup a hospital tank for him? I do not see worms in his poo (he doesn't poo that often), or hanging off of him.
