Figure eight puffer fish breathing heavily

1. Ammonia 0(but both tests came out milky) nitrite 0 nitrate 0 pH 8.2 SG .005
2. 20 gallon live planted with one figure 8 puffer
3. Usually snails/ frozen foods such as mysis shrimp/ brine shrimp. Usually do 2 25% wc a week. Use seachem prime.
4. I was treating him for IPs by soaking API general cure. I noticed very small red worms in the water and I also put in some of the powder into the water. They are still in the water. They're so small I never noticed them before. I have only given him two treatments of shrimp soaked in the general cure and that was yesterday and the day before yesterday. I did a 25% water change yesterday but, I noticed he started to breathe heavy and I actually did a second 25% water change that night and added in an air stone into his water. This morning he was at the top breathing really heavily and I also found him on the bottom just a minute ago.
5. The aquarium has been set up for a few months. I used Fishless cycling/ dr tims pure ammonia. When it was cycled I did a 90% water change and another ( the next day) right before I got the fish.

Unfortunately I found him passed away this morning.
