Very Sick Puffer

My figure 8 puffer seems really sick. She was lying on the bottom of her tank and not moving much this morning. The water hadn't been changed in a while, so my dad and I changed it when she first seemed sick. I checked ammonia (0) nitrite (0) and nitrate (around 5). I am working on getting her on a proper feeding schedule and water change schedule, since they haven't been very stable lately. No big changes in the last week, but I wasn't feeding her as much. She is the only one in her 27 gallon tank. Is there anything I can do to help her?

Do you know the parameters before the WC? What's the salinity?

SG is 1.004. I dont know the ammonia/nitrite/nitrate from before last water change, but my dad might know. He wont be home for a while, though. She is being really sluggish and just staying on the bottom. the water perameters are good now, and i think that she is suffering from nitrogen poisoning. She seems to have gotten worse since yesterday, though.
