I think my puffer has internal parasites

Parameters are fine SG 1.003. 20 gallons in a 29 gallon tank.

I have had him for a few months and he eats Ramshorns and bloodworms and his belly puffs up from being full but the next day he looks like he is going to die from hunger. Poop looks normal.
Tank mates are
2 mudskippers
2 red claw crabs
6 bumblebee gobies

What can I do to help this poor guy

Just bought hikari prazipro. I hope it works. I don't have a quarantine tank but I do have a small 4 gallon plastic tank I could set up. Should I just feed him in the tank?

You must treat all fish. Soak their food in it.
