VERY SICK PORCUPINE PUFFER!! HELP!

Hello
I bought a porcupine puffer about two months ago and he has been fine up until now.
He is in a 100 gallon, FOWLR tank, other tanks mates are 1 yellow tang, and a snowflake eel.
he is 4 inches, and all of a sudden he is not eating, he got very skinny, and he isn't moving much, he just stays at one part of the tank, also, usually if i net him he would puff up to a ball, and today when i did that to transfer him to a QT tank, he was only able to puff up very slightly...
All parameters are perfect,
what do you think it is? i am betting on worms but how would i treat that
