Worming with Kusuri + white sand cleanliness?!

So I just got some tank water into a beaker, put the right amount of Kusuri in and an earthworm mixed it all up, left it for a few mins and then put it in the tank. From other threads I've read, after 10-15 mins sometimes people see white worms coming out of the fish (grim) does this actually happen often?
I (stupidly maybe) did my weekly water change just before I put the wormer in, so do I have to suck the worms up if I see them, or use a turkey baster or something? O.o
Also I have white sand and its pretty much driving me crazy
Because of all the snails worms and other creepies that my puffer eats, there is so much crap on the bottom. I have a python and I suck it all up as best I can every water change and suck up any big bits with a baster during the week when I see them - but no matter how hard I try, there's always bits still on the sand. I think Im only noticing this more as the sand is white and everyone probably has this problem but Im not sure? Is it okay to have bits of stuff on the floor or is there some trick I'm missing to get it all gone?
