Treating puffers with ich

It is the salt that actually does the killing. The elevated temperature shortens the cycle of the ick, which allows us to kill it faster. The melafix soothes the skin of the fish.

i thought ich couldnt survive above 86 deg???

I'm not sure if that's true or not, but the speed of their life cycle does increase with a higher temperature. I imagine that a high enough temperature would indeed kill the ich, but it would probably also kill your fish.
The reason for increasing the temperature is simply to get them into free swimming form and into the gravel asap so that we can vacuum them up and remove them with water changes.

The heat, as LL and x0r said, causes the ich to hatch faster. If you shorten the life cycle, the ich will die quicker from the salt. Also I think the article is assuming the puffers are kept in pure FW, meaning no aquarium salt added. IME I have had little luck with using salt to treat ich, so I would just give them a half dose of an Ich treatment.
