I only ask because my puffer hasn't grown too much I've had him about a month n half he eats good, doesn't swim around alll that much seems happy 200 gallon tank sand substrate 0 ammonia 0 n02 10ppm n03 78* temp 35% wcc every 4 days feed 6. Days a week

It is always best to treat all new puffers for IPs, just in case. it doesn't hurt them.

Typically if an animal is from the wild they will have parasites. They don't have to have high parasite loads and the fish can still be healthy with some parasites. Since we keep our fish in captivity though most people like to treat for IPs as we don't want them in our tanks or fish.

Every wild-caught fish potentially has parasites. Parasites by definition are not beneficial. We expect our puffers to live >>10 years, potentially 20 or more, why not clean them up initially?

PP what would you recommend to treat puffers with IP's with?

I keep on seeing "add garlic juice" is it just simply adding a garlic powder in the mix or a fresh sliced garlic?

I chop garlic and chuck it in the feeding pot with warm water whilst defrosting food, then just pick the food out at feeding time.
