What do you have in there for decor? Can you describe the scratch? Puffers usually don't end up with deep scratches because their skin is so thick, open areas like that are usually infection related or disease related. Can you post Pics ASAP? In the mean time I'd add Melafix to the tank and an air stone as Melafix can decrease dissolved oxygen.

Taken this morning...

Decor in the tank are mostly pieces of driftwood that has been in there for many years with him.
There is a corner where its mostly large smooth rocks instead of gravel.
Other inhabitants are 2 cherry barbs

Hi Pufferpunk,
It refers to Beetroot or to the nasty looking patch?

But is that the original mark or is it growing, line an infection? What is in your tank that could have caused such an abrasion? Their skin is like thick leather, with tiny needles all over it.

My guess is that it charged real hard against the driftwood... there is nothing else in there except that for decor...
The patch started off as a scratch and now its like this.
Noticed that the forehead area was swollen before this nasty patch turned up.

Seems like a very bad bump into the wood or even the glass surfaces.

How big is his tank? Driftwood is too soft to cause this.

The tank is 4ft x 2ft x 2ft
120 gallons I think

The patch seems to have spread.

Only thing I can think of is the heater? How is it looking since you added the Melafix? Better or worse?
