We have had a MBU puffer for around 3 weeks and he is around 6 inches.
He had cloudy eye and we are treating it with Melafix. However, after checking the water, it was high in amonia, so we have been changed 25% water each day and now that seems fine. (we have had 4 other fish die over the past few days).
It is a 5ft tank and fish seemed to get on well before this all happened a few days ago.

My worry is that after changing the water in the evening, the morning after, he seems to look better. After giving him Melafix he seems unwell again, is it possible that it could actually be harming him?

What is your ammonia / nitrite level now? I think you may have put this puffer in an uncycled tank hence his health. Doing so will only stress him and adding melafix to the mix will only make things worse if your water quality is crap!
I would say more but I will not til I know Precisely the water chemistry of your tank, so please answer the questions above in RED!

40 Nitrate

His fin also looks a bit red.

Red fins = bacterial infection due to poor water quality. Same with the cloudy eye. There are many more Qs in red above, that need answering. Up those WC to 80% daily & clean the substrate, until you can straighten out the parameters & improve fish's health.

How many gallons is this tank? What are your immediate plans for upgrading?
