huge soft lump on right side by fin laying on side

my puffer is in a really bad way all of a sudden and is laying on his side like he is nearly died!!!
he has a large soft lump just behind his right fin
he is in a 8x3x3 tank with 4ft sump
natrate is0.1
amm o
nitrite 0
temp 27
doesnt look like i have much time he is 26" long and ive tried burping , if i put him near oxygen he seems to perk up but goes downhill fast and breathing slows right down
he also eats mussells in and out of shell prawns cockles in and out of shells
50% weekly water change
tank been running for 2yrs
