Dwarf Puffer not swallowing

I've had my dwarf puffers now for quite a few months. They have always been really good with eating frozen blood worms. In the last few days I have noticed one of the 4(the runt) is not really swallowing his food. He still seems to have the same appetite as always but he takes a worm in his mouth and I see him swim around with it hanging out of his mouth. Usually it seems like he spits it out eventually. He's getting really skinny. I thought maybe it was a change in the type of bloodworms so I just went back to the other brand that was smaller. But looks like it's more of the same thing. Any ideas? Other three are still eating a lot.

I had the same problem.
It could be IPs (internal parasites) but mine was inconclusive.
I did have another that started to get that way until my cherry shrimp started breeding in the same tank. He/she looks almost normal now.
If you can get pond snails just slightly smaller than their eye he/she may go for it. Mine only go for live food so keeping DPs is a real challenge for me.
ohhh BTW... my pond snails came from a medicated tank.

I would treat for IPs. The worms in side of him may be up to his throat.

did you find out what it was by any chance?
