mbu with what looks like a prolapsed gut!!!

I hope somebody can help - I've not been on here for ages but this morning I noticed that Barry (my mbu) was having a very red looking crap. On closer inspection it looks more like part of his gut! - To make things worse the pakistani knife fish tried to eat it and dragged it out further! Instantly I seperated him but he justs sits there now looking sorrowfull.
Will it go back of its own accord, has this happened to anyone else before? is there anything I can do?
Tank parameters are all normal nitrite 0, ph 7.2, nitrates 80 (bit high I know) ammonia 0
hope someone can suggest a course of action

<Please read your letter before sending, to be sure you have used the proper punctuation & capitalization. I corrected this one. These have to be posted in our FAQs. My thought about your puffer is that it may have a prolapsed rectum/intestine.
You can try treating with 1 tbsp Epsom salt/5gallon. It may just be a superpoo. Epsom salt wouldn't hurt in either case. You might be overfeeding your puffer. In that case, the food is only partially digested & this may be what you are seeing. Here is an excellent Mbu story by a puffer keeper of close to 50 years:

the pictures show the distension, I definately don't think this is worms it is definately a prolapse

I'm not sure if I can help, but I had a similar problem a few weeks ago with my suvattii. There was something coming from his anus and I thought it was his intestine. I really didn't think he would get better and prepared myself for his ultimate death.
Well I went ahead and treated him for worms, I used a wormer made for Discus, so that it is gentle for the puffers. I also added Melafix and Pimafix, but not at the same time as the wormer.
All I can say is that my suvattii is absolutely fine now, he's eating and doing things as he normally would do, there's NOTHING hanging out of his anus AT ALL.
So that might be the route for you to take ... I cannot say for sure because I am still very much a novice when it comes to ailments and health issues with my puffers.

That really does look like a prolapse. Like coldmachineUK said in the earlier post, I'd treat for IP's and also with eposm salt. She has the appearance of needing a round of IP meds, kind of pinched in and uneven around the midsection. I'd do the IP treatment and then, like was mentioned, treat with Pimafix and Melafix afterward.

Damn that is not good.
For parasites I have used Kusuri Wormer Plus but I dont think that'll do much but could be worth a try along with other treatments recommended
What also is springing to mind is perhaps send him under with some clove oil and try to gently push it back in. (I think i'd use that as a last resort) If medications dont seem to make it better i'd try and find a vet in your area that has done some work on fish.
Is he still passing waste?

Yeah, i'vehad good results from it, there used to be some stuff used on her called Flubenol 15 but we couldnt get it any more from the supplier. Kusuri is basically the same stuff. Flubendazole ( think I remembered the name right) based wormer.
Top stuff. Cleared up IP's on puffers no problem with no side affects. Used it on my Bichirs just incase they had any parasites lurking in the gills.

biggest problem now is trying to find a vet in my area who can help - fish vets seem few and far between any one know of one in Surrey/Kent/East Sussex?
On the plus side he is swimming now and not just sitting on the bottom, I spoke to a vet who used to treat fish who advised me that because of the pressures in a puffer fish gut simply pushing it back in won't work, it needs to be stiched in but then that may cause problems with any fecal matter which gets stuck - prognosis not good
