Thin white worms

This morning while observing my 2 feet (56 liters) puffer tank, I saw a thin white worm swimming in the tank and there is also a circular white stuff with a lot of tiny tentacles or feelers moving across the surface of the tank.When to my room to grab my camera phone but by the time I got back they are all gone.
Anyone know what are they? And are they harmful to my arrowhead?
Details: Freshwater, planted with some java moss on a coconut, a Crinum calamistratum, six Pygmy Chain Swords and a small Anubias. Salvinas and Frogbits covering 2/3 of the tank surface. Using 55 watt, fertilizer and added liquid carbon just yesterday. No water change for the past 2 weeks.
