sick puffer

we have had a puffer in a tank for about 2 weeks. every thing looked good but when i came home one day he had strange bubble on his under side. it doesn't look bad, it almost looks like she is pregnant.. i have a spiny box puffer fish please... old
i took a picture of the fish.. we stured up the tank so im waiting a few days to retest the tank. but i attched a picture of the tank
