SAP with lump

pH around 7/8 - tap water
Ammonia, Nitrite - 0
Nitrate - 10-20ppm
100L tank with 2 SAPs and 5 otos
I did a water change yesterday, 50%, but had been away for 10 days before, so the tank didn't get changed for 2 weeks, usually it is 50% weekly.
When I came back from holiday yesterday, all was well except that one of the SAPs (the one that is always a bit thinner and smaller and stupid with food), had a lump just below his lip. I thought that it was just a cut that I may have caused from trimming their teeth before I went away, so I added melafix and pimafix after the water change.
I came back this evening to add some more and feed and it now looks well, like a pink spot with a white head basically.
I am thinking that this is maybe a cist that is going to burst, so is there anything that I can do except add melafix and pimafix and just hope for the best??
