For ferts, I use the EI Method of dosing, KHO3 and KH2PO4 for macros and CSM+B for micros. It is pretty much the staple for an high-tech planted tank. In depth here:

For lights I use a 2x 24watt T5HO bulbs, one 6500K and one 10,000K.
For substrate I use ADA Aquasoil Amazonia. Love it and wouldn't ever go back to anything else.
For CO2 I have a 7lb Co2 tank hooked up to a regulator, then a solenoid, then needle valve, leading to an inline ceramic diffuser on the output of the canister filter.
