Could be a bite. It's not ich because he would have it all over. Perhaps a fungus. Puffer Queen would know more about that. Has he eaten since you brought him home?

he is going up and down, side to side really aggressively. i know he's getting used to my tank. and yes he ate 1 gut loaded shrimp

Well pacing is normal and it's good he ate. You can try some Melafix if you think it's a bite. Or wait to see what the PQ says. Do you have a full tank picture?

Keep a close eye on your water quality. If he's looking healthy, let's keep him that way. Get a system down for how you are keeping your pH steady and to reinterate what pufferpunk stated, try to find out what is lowering your pH.
I once had an issue with decor that severly lowered my pH. It's amazing what they make that is "supposively" suitable for fishtanks. That would be a good place to start... Any rocks included in your tank? Another possibility if you have some in there.
If you do have rocks in there, a good way to test that (or decor) is to get a bucket big enough to hold the decor/rock. Test the water before you put it in and then put in rock/decor to sit a day or two and then test the pH again.
