Do try the decorations. It will help your puffer feel more secure about it's surroundings too.
It may very well be sick if its not eating well. What are the water parameters and how did you cycle the tank?
BTW, recommended tank size has little to do with the size of the puffer. Relatively speaking, puffers have a very high bioload and therefore require much larger tanks than other fish of the same size. There's also the matter of movement. GPSs get to be 6 inches long and need a good bit of room to swim around. Keeping a fish in too small of a tank to handle the bioload will cause stunting, which causes all sorts of health problems and leads to premature death.
I hate to say it but if you can't get a bigger tank for it in the near future, you should probably try to rehome or return him.
