The gill clamping is something puffers will do occasionally even when in full health. If you think it is parasites you will need to food dose. As he is not eating you may need to tube feed. Before attempting a tube feed i would try injecting an earthworm with prazi and dangle it infront of him. Someone else should comment on the dosing amount as i do not know with a puff this large.

Be sure to add more turbulence to the water surface, either by a powerhead across the top or an airstone bar. He's having enough trouble breathing & Melafix makes that worse.
