I do agree that it is not easy to get 0 nitrate with messy eaters and large fish in SW (or FW) but it can be done. I wish I had already been well along in testing plenum tanks when I had the T. mbu. Anything would have helped.
Would you recommend keeping a refugium with a DSB or an algae lit refugium with saltwater puffers?

Why not both? That's what I have. It's plumbed in next to the tank.

consider the sump a mechanical garbage can where wastes are deposited and removed. A fuge (refugium) is usually there to hold aquatic plant life and other living organisms that help clean and oxygenate the water.
There is no reason why this can't all be in one system but people prefer to have the dirty work hidden away from the display tank.

UPDATE - puffer's eyes are much clearer, he's eating frozen jumbo shrimp again.... my guess is eyesight is about 80% back to normal. He can definitely see us thru the glass - gets very excited especially when my ten year old walks in the room. Still has not taken a shrimp hand held at the top of the tank like he used to. We have to use feeding tongs to get the shrimp near his face. I'm encourgaed that he'll continue to improve and be back to 100% in another month or so.

that is good news!!
it is a shame that sickness and injury is so sudden and the healing process takes such time.
