I'd take the water to a LFS and have it tested if you can't buy a good test kit yourself. You need to know what's going on in order to help this puffer. As Flutter stated, water changes are always good because if there is anything in the water that's causing the puffer to be ill, it will be diluted, which is what's needed. I'd be doing water changes if it were me, and you might also add an extra airstone to get more oxygen into the water.
