Looks like he's got some excessive slime coat and other physical problems, probably secondary to the ich. I'd keep treating the ich and see what problems remain as it goes away. I had a very sick GSP that looked much like that once, but with a high SG water and lots of changes, his ich went way and the other problems did shortly afterwards.

just , quarantine the little puffer all by himself and raise the salinity every two hours by a teaspoon per gallon , until it becomes high end brackish raise the temperature to 31 celsius and keep giving the little puffer full marine salt baths every five hours for 2 minutes. he should recover in two days if you keep doing water changes and replenishing the lost salt. you might lose the bacteria in your filter, so be alarmed for that

i find giving them their favorite food helps, (if they're eating) but i also find increased arration helps alot as it lets salts go through the gills more as more oxygen is taken in which heals them faster. just my experience.

FW/SW baths should be done for 30 minutes minimum or they are useless.

Based on our experience using FW dips, we think 30 minute dips would kill a fair percentage of the fish treated. We stopped using FW dips at all because even hardy tangs, triggers, and puffers seemed to experience enormous stress and long recovery times after dip from 2-10 minutes.
