I have one 2" green spotted puffer in 1.012 salinity with no tankmates.
I heard fresh water ick can't live in brackish, so I try using med for Ocean fish ick.
It has been a month now and there are no change, it is not spreading but not go away either.
He didn't seem to irritated by it at all unlike the time that he had ick in the past(when I first got him)
So can it be other parasite?
I'm think things that look like white spot on his belly is just his spike? Please correct me if I'm wrong.

Your puffer looks very healthy! I would hesitate to use ANY meds, especially for just 1 spot. Definitely will do more harm than good. Do a large WC & filter with carbon to remove the meds.
Doesn't look like ich, prob just an irritation from something. I suggest getting that SG up.

You can go as high as SW & then add live rock & a protein skimmer but high-end BW (up to 1.018) is good, you'll just have to continue doing 50% weekly WC on that.

If I'm going full marine. Can the marine bacteria generate on their own? Or I really need a specific bacteria that come from live rock?

There are more odf the white spot on him now. On his bottom fin and on the left fin. The one on the left fin is clearly visible.
The salinity of the tank is now 1.016 still slowly raising to 1.018.
The only time that I see his belly turn a little bit grey on the side is ahen he eating.

How large are the spots? Like a grain of salt or larger?

The one on the bottom is just like the one on upper fin the first pic. I try to post pic on my other reply but it doesn't show, so I edit it and now it disappeared. Hoping that when this reply is up that one will also have picture.

I do see the picture. Other than that, your puffer looks very healthy! I would just keep an eye on him. I have seen spots like this before & they went away on their own.
