What are your water params now? He doesn't look excessively skinny.
It's amazing how easy maintenance is. If done regularly and thoroughly

Those little white circles look like bites from other puffers. It's pretty common with new puffers.

Are those little white specks sand, or do they not come off?

Recall the face of the poorest and the weakest man whom you may have seen, and ask yourself, if the step you contemplate is going to be of any use to him or her. Will she gain anything by it? Will it restore her to a control over her own life and destiny? In other words, will it lead to freedom for the hungry and spiritually starving millions? Then you will find your doubts and your self melt away."

I know this may sound strange but can we get a head shot while feeding?

The reason I ask is that almost looks like a baby ceylon but my ID skills are not the best. Then again, LFS' are also known to mislabel puffers.

I personally would start a heat/marine salt treatment for ich now while waiting for the IP meds since it will take a couple days for it to arrive. That way you we can check off two possibilities in a shorter amount of time and thus giving us better chance to find out the problem before it gets worse.

Do we know it's ich? Maybe I just totally missed it, but I thought there was no confirmation from Petoj87. Sometimes puffers do just get sand on themselves, and it comes off when they move around.

Petoj87, are those white spots permanent or were they just sand?

I have already started adding marine salt to convert the tank to a brackish conditions i don't know my current salinity as my refractometer does not show the lower scales..
Also the fish seem to hover under the filter for extended periods of time is that normal?
