Sick T. Miurus

I came home from vacation to find my T. Miurus looking very bloated with its mouth looking swollen and damaged as well. I’ve also noticed it’s eyes are very dark in coloration(almost solid black). My dad was feeding the fish red wigglers every 3 days. The tank size is 30 gallons with pool filter sand as my substrate that has been set up for at least 10 years. The water parameters are: PH 7.4-7.6 range, ammonia 0 ppm, nitrite 0 ppm, nitrate 20-25 range. I use an api master test kit for my water testing. He is the only inhabitant of the tank but I did put 10 ghost shrimp in before I left for vacation for him to feed on. I did a roughly 75% water change yesterday and I’ve given the fish 2 methylene blue baths. Wondering if there is anything else I can or should do to help my fish.

How long were you on vacation? Was anyone watching him for you? What is in there that could have damaged him?
