My South American (the nice puffers) are dying

Two of my South American puffers have died within the last mouth. I did tests both times they died and came up with nothing. It’s a 75gallon. Ammonia is 0 nitrite is 0 nitrate is 5 GH and KH are 71.6 PH is 7.8. It’s a planted tank with sand substrate. I have two sponge filters and large hang on back filter. They live with 8 ember tetras, one 2in blue phantom pleco and one 3-4 in king tiger pleco. I quarantined the puffers embers and plecos with general cure, ich x, and erythromycin and followed the packages. I do about a 40% Water change every two weeks. They’re fed rams horn snails and blood worms daily. Does anyone know why they’re dying? Anything I can try to help them?

No external issues? They are very sensitive to velvet disease.
