Dwarf pea puffer is skinny

I got a dwarf puffer about 2 weeks go, It is very active but I recently noticed it is looking very skinny. At first I thought it was because of the young age but I don’t think so anymore. It has a big head with a skinny body. It seems to have an appetite trying to eat food but it’s almost like it can’t swallow. What might be wrong?

black worms, small snails, cherry red shrimp, Mys shrimp. Dwarf Puffers are notoriously difficult to feed. You can try to medicate some of the food to treat for parasites, but I have a feeling that isn't the issue.
