His vigor and appetite are good and so far I have not done anything.
I try to do 50% water changes very week but it's been more like 17 day since hte last one and he will get one today.
Ever since those changes at least every 20 days but more often sometimes water quality has been pretty stable. He also has some live plants and algae to absorb some of the waste.
He does tend to bump the side of the tank.
I have shale in the tank because I read that some puffers use to maintain teeth but I am going to replacement soon. I have not seen him use them for that. He does eat crayfish and he may have had a pinch when feeding.
It originally looked flat like a wound but is now more bulbas. Maybe it is healing normally?
Tank is set up for 2 years and no recent changes.
I did at some point remove the screen on his canister filter intake. This is a bit of an issue because he sometimes gets stuck to it briefly because of the suction. He seems to have learned tho not to do that. I've misplaced the screen.

He is getting a bigger tank soon! I have an FX4 and am planning to get a 120g tank within weeks.
I am a bit worried about transistioning to the bigger tank ... but i think he will be fine. I will do water changes in his current tank 3 times and start the new filter with 50% ceramic media from old filter. That media has been in place since start of current setup.

Water changes really must be consistent at 50% weekly. It does appear to be a pinch from a crayfish. Please do answer ALL the Qs above, in red.

More info from my British puffer expert:
Bacterial infection. Meth blue baths preferably to avoid the development of fungus. Big water changes and a mild antibacterial. You can also try Melafix.

Unfortunately, it doesn’t seem like the states have a very wide selection of good antibacterial that are not antibiotic based or complete snake oil. I think we need to put our heads together at some point to find some decent meds people can get hold of in the US.

Personally, I’d take the substrate out and leave bare bottom until healed. Replacing the substrate with sand when treatment is done.

Waterlife Myxazin is very good.

I did water changes and fortunately his immune fought off infection without the antibiotics. He has a sand-like substrate in his current tank and will have a mount of "caribsea super natural natural sand" from in his new tank. More on that in another post.
