Burn? Scrape?

Hello all! I checked on my little dude this morning (Congo Puffer) and noticed a burn or scrape on his lip (pictures to follow). I put some Stress Coat with Aloe Vera to help it heal, but is there anything else I can do? He's not burrowing (probably because his lip hurts) and I have no clue how he got the injury! There's nothing sharp in the tank and he was just fine yesterday. Any ideas? Tank information as follows:
1) Water Parameters: (as of this morning)
Kh: 2
Gh: 6
Ph: 7.3
Ammonia: 0 ppm
Nitrite: 0 ppm
Nitrate: 10 ppm (tank is full of live plants)
2) Tank size and a list of ALL inhabitants: 40 gallons with Congo Puffer and a few small molly fish he eats.
3)Feeding, water change schedule and a list of all products you are using or have added to the tank: Feed shrimp two or three times a week, 50% water changes once a week, Stress Coat and SeaChem Prime
4) What changes you've made in the tank in the last week or so. : None? I keep everything as constant as possible.
5) How long the aquarium has been set up, and how did you cycle it? Aquarium has been set up for 6 months now and it was seeded and cycled before we got the Congo Puffer.
I'd appreciate any help! We adore our little dude and want him to be healthy and happy! Thanks!!!
