Sometimes predators trying to eat a puffer die. Mostly when the puffer puffs while behind the teeth and already in the throat.

But I've seen such an occurence as above, which went bad for the puffer. Morays have long teeth in the middle of their upper jaws. They can pierce the puffer skin with a little luck (for the eel that is) and leave a hole in the organ they keep the water in (stomach in porcupine puffers, different for tetraodontids). So, the water is removed quickly and the puffer was swallowed. Here's another one. https://www.youtube.com/watch?v=E45JMl2JF4w

The stomach contents of one moray species (Gymnothorax ocellatus) sometimes contains puffer remains, which I find interesting, because they are among the few species, that don't have those piercing teeth described above and I'm not sure how they do it. On the other hand they have exceptionally large mouth compared to other eels of similar size.

It's better not to keep large morays with puffers. The one on the first post can definitely eat a puffer of that size. As a side note, the morays are most likely immune to the toxin.
Give a man a fish, and he'll eat for a day. Give a fish a man, and he'll eat for weeks.

There are predators which are not sensitive to puffer toxins. Sharks generally are immune. I don't remember about morays.

But why was a food-sized puffer in with a big moray? Or is that a wild shot?

A wild shot, not captive. And the puffer got away...
