You may just have a single bully on your hands and need to keep them separated. The tank should be densely planted to remove lines of sight. Do you know the ratio of male to female?

Those are puffer bites. Amazon puffers prefer to be in groups of 5 or more.

Sorry it took me so long to get to this post! Having computer problems… if your tank is cycled you should be showing some nitrate. How exactly did you cycle the tank?

Your tank looks decorated well but this species is best kept as a singleton or in groups. Longer footprint is always best. How about Pea puffers instead?

I'm not sure why you're using an anti fungal. I would be using Prazipro to treat internal parasites. Best treated internally by soaking their food, you could try adding garlic to enhance appetite.
