I'd definitely recommend at least a ten gallon, because it'd be easier for you to take care of really, then you can get two DPs, as well. 10 gallons are really easy to come across too. Tank conditions can really matter as RTR says.
You could try cutting the worms up though. I have two eels who I chop worms up for, because they won't eat them whole. Also, his "teeth" aren't over grown, are they?

Why not try crays.. They are more fun to watch as well. I wouldn't breed anything in the same tank as your snails, because they're likely to eat the snail eggs. Figure 8 puffers are low-end brackish fish, who do best with an SG of 1.003-1.006. The lower end of that range is when using aragonite sand, which you already use. Like other said, feed him a bit more, and feed more variety. He needs way more than just shrimp. Also look for a liquid vitamin to soak his food in before feeding it to him.

Were you doing a water change or anything at the time? Sometimes temperature fluctuations will cause a heater to bust like that.

Look into a tight fitting lid, because those things love to climb out of the tank and go carpet surfing.

Just wanted to ask, is your heater adjustable? Not having an adjustable heater was the entire downfall of one of my friend's tanks a year or so ago.
